Kate serves as a Care Manager for Advanced Care Management Services (ACMS), where they provide direct advocacy, comprehensive resource coordination, and tailored care planning for clients. Driven by a passion for health equity, Kate works closely with individuals and families to dismantle barriers to care, manage complex transitions, and empower clients to achieve long-term health and wellness. Areas Served: Lancaster, Lebanon, Schuylkill, and Chester Counties.
