CAPABLE is a tested program developed by Johns Hopkins School of Nursing more than 15 years ago and is being disseminated nationally. It is brought to you and your community by UDS.
Helping Older Adults Live Safely and Independently at Home
CAPABLE is an evidence-based program that helps older adults improve their independence, safety, and quality of life while continuing to live at home. Working together, an Occupational Therapist, Registered Nurse, and handyperson help participants achieve the goals that matter most to them—whether that’s bathing safely, preparing meals, moving around the home with confidence, or continuing to live independently.
Over the course of four to six months, participants receive personalized support, practical strategies, and minor home modifications designed to improve daily function and reduce barriers at home.
Who Is CAPABLE For?
CAPABLE may be a good fit for older adults who:
- Have experienced a decline in health or mobility
- Are finding everyday activities more difficult
- Could benefit from minor home modifications or adaptive equipment
- Want to remain living safely and independently at home
Eligibility
Participants must:
- Be age 60 or older
- Live independently in Lancaster, Lebanon, Berks, or York counties
- Have difficulty with daily activities such as bathing, dressing, walking, or meal preparation
- Be able to actively participate in a four- to six-month program
- Be motivated to work toward personal goals

Yes! CAPABLE does not have insurance or income requirements at this time
Yes, but it depends on the type of home health services you’re receiving. If you’re currently receiving home health Occupational Therapy (OT) or Registered Nursing (RN) services, you can begin CAPABLE after those have ended. If you’re receiving home health Physical Therapy (PT) only, you may be able to start CAPABLE right away.
Yes, CAPABLE focuses on ways to make things safer and easier for Yes. CAPABLE focuses on making everyday activities safer and easier. Even if you’re able to do things on your own, you may benefit from the program if daily tasks feel like a struggle or you’ve noticed they require more effort than they used to.you in your home. Even if you can do things but it feels as if it’s a struggle at times you may benefit from CAPABLE.
No. CAPABLE is designed to help you live safely and independently at home, but you do not need to be homebound to participate. In fact, we encourage participants to stay active, engaged, and connected with their community whenever possible.
