They will be there every step of the way to help you assess, plan, coordinate and manage every aspect of your or a loved one’s care.
Customer Care Representatives provide access to reliable in-home care, accessible home modifications, and specialized equipment designed to make your life easier. They can also help you create an independent living plan, access financial assistance, and guide your progress for as long as you need them.

Person Centered Counseling is a free service that connects Pennsylvanian’s to public benefits and private resources. A knowledgeable and compassionate person-centered counselor will:
- Identify long-term services and support options available in your county
- Provide guidance and help secure services that best fit your needs and preferences
- Follow up to make sure that everything is going well
