Jen will conduct comprehensive skin assessments in the home and recommend appropriate prevention and/or treatment interventions in collaboration with the Service Coordinators, in home health professionals and treating physicians. In addition she will track and trend all necessary data to ensure that care is provided and delivered in a quality, timely, and respectful manner that is consistent with the mission, goals and objectives of UDSF and the Home and Community Based Services Program.
